Revenue leakage is rarely caused by one dramatic failure. It usually develops across small handoff gaps: incomplete registration, unverified coverage, delayed charges, missing documentation, and denials that are corrected without addressing the cause.
A useful review starts by mapping ownership. Who confirms eligibility? When are charges reconciled? How quickly are rejections corrected? Which denial categories are trending? How is aging work prioritized?
The goal is not simply to work harder. It is to create a visible operating rhythm in which exceptions are identified early, assigned clearly, and reviewed for recurrence.
